Dr James Klinect

Dr. James Klinect

Dr. James Klinect is the Founder and CEO of The LOSA Collaborative. He received his Doctorate at The University of Texas at Austin where he served as the project manager, leading the development of TEM and LOSA with his mentor Dr. Robert Helmreich.

 

James has authored and co-authored several publications and presentations, including the aviation industry’s recognized source material on LOSA (ICAO Document 9803 and FAA Advisory Circular 120.90). His LOSA and TEM research is regularly cited and used in academic journals, regulatory guidance, airline safety management systems and pilot training curriculum.

Stephen Ingham

Stephen Ingham

Stephen Ingham joined The LOSA Collaborative in 2002 as an external observer and has conducted over 1000 observations in more than 50 airline and helicopter operators.

 

Today, Stephen leads The LOSA Collaborative’s Rotary-Wing and Aero-Medical LOSA programs and is widely recognized as an industry thought-leader on LOSA and TEM. Stephen is also involved The LOSA Collaborative’s Airline LOSA programs, especially throughout Latin America as a proficient Spanish speaker.

 

Before The LOSA Collaborative, Stephen was the owner of an aerial work company where he spent more than twenty-seven years aerial spraying, including night operations and geophysical surveys. He also served as the Air Safety Chairman & Regional Chairman for the Honourable Company of Air Pilots.

Hugh Webbon

Hugh Webbon

Since joining The LOSA Collaborative in 2006, Hugh has run multiple LOSA projects for airlines and implemented Threat and Error Management training for both Instructors and pilots.

 

Hugh continues to implement LOSA programs and serves as The LOSA Collaborative’s Director of Strategic Alliances where he leads our partnership with CAE.

 

Before The LOSA Collaborative, Hugh enjoyed a 40-year airline career with British Airways and Singapore Airlines, holding Union and Management positions as well as IRE/TRE and Check qualifications.

Since 2001, The LOSA Collaborative has steadily grown into the gold standard for LOSA implementation in the aviation industry today. None of this would be possible without our team of dedicated collaborators.

 

 

It is a testament to our success that many of the people involved in the early days of LOSA and TEM development are still with The LOSA Collaborative today. It is our passion, and for many, their life’s work.

 

 

Our team of over 20 people around the world is an eclectic group of aviation experts with distinctive experience and various flying, data analysis, training, and management backgrounds.

Our Corporate History

Beginning in the mid-1990s, Dr. James Klinect, our founder and CEO of The LOSA Collaborative, helped start a movement that forever changed aviation safety.

LOSA & TEM - The Beginning

The foundation of LOSA and TEM thinking emerged from ideas incubated at The University of Texas Human Factors Research Project under the leadership of Professor Robert Helmreich. Dr. Bob, as so many affectionately remember him, was instrumental in the conceptual development of Crew Resource Management (CRM) in the early 1980s and 1990s. Bob and his team operationalized CRM training curricula and assessment tools, including the first observational audits of CRM performance conducted during normal flight operations. Building on the lessons learned from these audits, James Klinect as a Ph.D. graduate student and his colleagues expanded the scope of observations beyond CRM to enable a deeper understanding of what pilots do to fly safely from Point A to B. It was this change in thinking that led to the birth of LOSA and TEM.

Building Trust: The Beginning of The LOSA Collaborative (TLC)

From the inception of LOSA, it was clear that the sound methodology developed in the research was necessary for observations to be practical. Still, the execution was equally important, mainly when it came to building trust. Trust in who does the observations, trust in data quality, trust in who conducts the analysis, trust in how the data will be used, and trust that data will be kept confidential and anonymous. As more airlines started to become familiar with TEM and LOSA in the late 1990s, it became clear that they were hesitant to conduct a LOSA without assistance. Data quality concerns were significant and continue to be so today. Airlines wanted support, and it needed to come from a trusted source. In response to these industry demands, James founded The LOSA Collaborative in 2001 with an endorsement from The University of Texas as a private and independent organization. Earning industry trust is at the center of everything we do at The LOSA Collaborative and is something that we continue to protect today with every LOSA that we implement.

The LOSA Collaborative is focused solely on LOSA and TEM services, and we continuously review and refine our processes in response to customer feedback and suggestions. We are an award-winning company driven by the passion and expertise of our people and united in one purpose, vision, and mission.

Our Awards

2014

Flight Safety Foundation (FSF)-Airbus Human Factors in Aviation Safety Award The Flight Safety Foundation–Airbus Human Factors in Aviation Safety Award was established in 1999 to recognize “outstanding achievement in human factors contributions to aviation safety.” The award was instituted to encourage human factors research that would help reduce human error — one of the most common elements in aviation accidents.  The award — instituted by the Foundation and sponsored by Airbus — is presented to an individual, group or organization for a one-time contribution or sustained contributions in the field of human factors.

2011

The LOSA Collaborative accepting an award

The Cumberbatch Trophy is awarded by the Honourable Company of Air Pilots for outstanding contributions to aviation safety.

 

The trophy was originally commissioned by Miss Alice Cumberbatch in 1936 and is awarded to an individual, group, or organization for an outstanding contribution to air safety.

Our Timeline of Milestones

1996

Continental Airlines implements the first LOSA based on Threat and Error Management (TEM) and is recognized as the first LOSA conducted by The LOSA Collaborative.

1998

The TEM framework is presented to the public for the first time at the International Symposium on Aviation Psychology in Columbus, Ohio.

1999

ICAO endorses LOSA as the primary tool to develop countermeasures to human errors in aviation operations and makes it the central focus of its Flight Safety and Human Factors Program (Source – ICAO Document 9803).

2000

Continental Airlines becomes the first follow-up customer for The LOSA Collaborative, determining the effectiveness of change introduced as a result of the first LOSA.

 

Continental Airlines introduces the first known TEM training course to their pilots.

2002

ICAO recognizes LOSA as a Standard and Recommended Practice (SARP) with the publication of Doc 9803 – Line Operations Safety Audit.

2004

Asiana Airlines implements the 25th LOSA conducted by The LOSA Collaborative.

2005

Threat and error management (TEM) is recognized and adopted by ICAO in Annex 1 Personnel Licensing and Annex 6 Operations of Aircraft.

2006

LOSA is formally recognized by the FAA as a Voluntary Safety Program with the publication of Advisory Circular 120.90.

2008

The LOSA Collaborative conducts its 50th LOSA with Mexicana Airlines.

2010

Air Methods becomes The LOSA Collaborative’s first HEMS client.

 

The United States Air Force (USAF) Air Mobility Command becomes The LOSA Collaborative’s first military organization to implement a LOSA.

 

Delta Airlines implements our 75th LOSA project.

2011

The LOSA Collaborative is awarded the Cumberbatch Trophy by the Honourable Company of Air Pilots at Guildhall in London, England.

2013

The 100th LOSA conducted by The LOSA Collaborative occurs at Singapore Airlines – it’s their third LOSA overall.

 

The LOSA report for EBT gets published by IATA.

2014

The LOSA Collaborative is awarded the Flight Safety Foundation-Airbus Human Factors in Aviation Safety Award in Abu Dhabi.

2015

HNZ Group (NZ) is The LOSA Collaborative’s first offshore helicopter company to implement a LOSA.

2016

The LOSA Collaborative surpasses 25,000 LOSA observations collected.

2017

The LOSA Collaborative and American Airlines partner to implement the first accredited Continuous LOSA program.

2018

The LOSA Collaborative completes the first Remote Piloted Aircraft (RPA) LOSA with the U.S Air Force Air Combat Command

2019

The LOSA Collaborative completed its first Dispatch LOSA at American Airlines

2020

CAE and The LOSA Collaborative form a strategic partnership that enhances the link between LOSA findings and competency-based training.
